Wound Care Nurse Practitioner (NP) Position Summary
The Wound Care Nurse Practitioner is responsible for providing comprehensive wound care services across the continuum of care, including assessment, diagnosis, treatment, follow-up, and patient education. The provider will collaborate with an interdisciplinary team including physicians, nursing staff, rehabilitation services, and home health to promote optimal wound healing outcomes, reduce complications, and prevent hospitalizations.
Care may be provided in outpatient clinics, inpatient units, long-term care facilities, and/or via telehealth depending on organizational needs.
Primary Responsibilities:
- Perform comprehensive wound assessments and develop individualized, evidence-based treatment plans
- Diagnose and manage acute and chronic wounds, including:
- Diabetic ulcers
- Pressure injuries
- Venous and arterial ulcers
- Surgical and traumatic wounds
- Burns
- Ostomy-related skin conditions
- Perform wound debridement within scope of practice and clinical competency
- Order and interpret diagnostic studies such as labs, imaging, vascular studies, and wound cultures
- Prescribe medications, wound care dressings, and advanced wound care therapies as appropriate
- Apply and manage advanced wound care modalities, including:
- Negative pressure wound therapy
- Compression therapy
- Biologic dressings
- Skin substitutes
- Collaborate with physicians and specialists on complex wound cases
- Provide patient and family education on wound prevention and management
- Maintain accurate, timely documentation in the electronic medical record (EMR)
- Participate in quality improvement, infection prevention, and wound care program initiatives
- Support regulatory compliance and adherence to evidence-based clinical standards
Required Qualifications
- Current Nurse Practitioner (NP) license in the applicable state (or eligibility to obtain)
- Board certification as Family Nurse Practitioner (FNP), Adult-Gerontology NP (AGNP), or related specialty
- Current Basic Life Support (BLS) certification
- DEA registration (if prescribing controlled substances)
