A mission-driven Federally Qualified Health Center in Eastern Connecticut is seeking a compassionate Internal Medicine Physician to join its integrated primary care team.

This full-time opportunity is designed for a physician who wants to provide comprehensive, person-centered care while developing meaningful, long-term relationships with adult patients. The incoming physician will diagnose and treat acute illnesses, manage chronic conditions, provide preventive care and coordinate services across a multidisciplinary healthcare network.

Responsibilities

• Provide comprehensive, patient-centered medical care
• Diagnose and treat acute illnesses and chronic medical conditions
• Review medical records, examination findings, reports and diagnostic results
• Develop individualized treatment and care-management plans
• Communicate clinical findings, recommendations and treatment plans clearly to patients
• Provide preventive screenings and routine health maintenance
• Educate and counsel patients regarding self-care and disease management
• Coordinate referrals to specialists and other healthcare services
• Follow up on referrals, testing and recommended treatment
• Collaborate with primary care, behavioral health, dental, case-management and supportive-service teams
• Participate in interdisciplinary case conferences
• Identify and respond appropriately to high-risk patient situations
• Make required notifications to appropriate protective or community agencies
• Participate in clinical quality, risk-management and operational-planning initiatives
• Support programs developed in response to community health needs, epidemiological trends and healthcare-access challenges
• Participate in an on-call rotation
• Provide backup coverage for other primary care physicians when needed
• Provide acute-care services when appropriate and when maintaining the required hospital privileges
• Maintain complete, accurate and timely clinical documentation

Qualifications

• Doctor of Medicine or Doctor of Osteopathic Medicine degree
• Current or eligible Connecticut physician license
• Valid DEA registration
• Current BLS and ACLS certifications
• Board certified or board eligible in Internal Medicine or an employer-approved primary care specialty
• Ability to maintain hospital admitting or clinical privileges when required
• Ability to satisfy applicable payer, accreditation and organizational credentialing requirements
• Strong interest in community health or public health
• Experience with patient-care management
• Commitment to person-centered and culturally responsive care
• Ability to collaborate effectively within an integrated clinical team

Practice Environment

The physician will work within an integrated community healthcare organization that provides access to primary care, behavioral health, dental care, case management and other supportive services.

The practice emphasizes prevention, continuity of care and collaboration across clinical disciplines. The physician will primarily provide adult primary care while helping patients navigate specialty services and community resources.
