This provider will deliver comprehensive, patient-centered care with an emphasis on preventive services, chronic disease management, and women's health. Responsibilities
- Conduct comprehensive patient assessments, including medical histories and physical examinations.
- Diagnose, treat, and manage acute and chronic health conditions.
- Order, perform, and interpret diagnostic tests and screenings.
- Prescribe medications and treatments in accordance with state regulations and organizational policies.
- Provide preventive health services, screenings, and wellness counseling.
- Educate patients regarding disease prevention, medications, treatment plans, and healthy lifestyle practices.
- Develop individualized care plans based on patient needs and evidence-based guidelines.
- Collaborate with physicians, nurses, behavioral health providers, and other healthcare professionals to coordinate care.
- Document patient encounters accurately and timely within the electronic health record (EHR).
- Participate in quality improvement initiatives and promote positive patient outcomes.
Qualifications Required
- Graduate degree from an accredited Nurse Practitioner program.
- Current Nurse Practitioner license in the state of practice.
- Family Nurse Practitioner (FNP) certification.
- Active DEA registration and prescription authority.
- Strong clinical assessment, diagnostic, and treatment planning skills.
- Knowledge of evidence-based practice standards.
- Proficiency with electronic medical records and clinical documentation.
- Excellent verbal and written communication skills.
- Strong organizational and time management abilities.
- Ability to maintain patient confidentiality and professional boundaries.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
Preferred
- Experience in primary care, women's health, reproductive health, or family planning.
- Experience managing chronic diseases and implementing health promotion initiatives.
- Knowledge of preventive care and public health principles.
- Experience caring for diverse, underserved, or vulnerable populations.
- Familiarity with quality improvement initiatives and outcome measurement.
- Experience conducting telehealth visits.
- Existing credentialing with payors such as Highmark, UPMC, Cigna, or Geisinger.
Schedule & Compensation
- Full-Time
- Monday through Friday
- 8:00 AM to 5:00 PM
- Patient volume: Approximately patients daily
Compensation & Benefits
- Competitive salary ranging from $95,000 to $115,000 annually.
