Setting: Employee Wellness Clinic

Cases: routine wellness care, chronic care management and urgent visits

Schedule: Mon-Fri 8am-5pm with staggered 1 hour lunch (open to 4x10's or 5x8's)

Travel lodging can be provided if over 50 miles from clinic
