Seeking a Family Medicine Physician to join an innovative healthcare organization that is transforming primary care through a patient-centered, relationship-based, proactive, and team-oriented model of care.

JOB RESPONSIBILITIES
• Provide comprehensive Family Medicine care across the full spectrum of practice, from pediatric through geriatric patients.
• Prenatal care is preferred but not required.
• Diagnose, prevent, treat, maintain, and rehabilitate patients as a senior attending physician.
• Provide direct patient care in outpatient clinic, urgent care, and hospital settings.
• Prescribe medications, therapies, vaccinations, and other medical treatments.
• Order, perform, and interpret diagnostic tests.
• Maintain complete and accurate patient medical records.
• Monitor patient progress and adjust treatment plans as necessary.
• Explain procedures, diagnoses, and treatment plans to patients.
• Collaborate with nurses, pharmacists, social workers, and other healthcare professionals.
• Serve as a resource for fellow providers within the primary care clinic.
• Participate in medical staff committees and organizational initiatives.
• Participate in quality, safety, and patient engagement activities.
• Function as a member of the medical staff, including emergency department and hospitalist responsibilities.
• Perform additional duties consistent with training and experience.
QUALIFICATIONS
• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Graduate of an accredited U.S. medical school or equivalent foreign medical school meeting ECFMG requirements.
• Active and unrestricted medical license in any U.S. state recognized by the Federation of State Medical Boards.
• Completion of an accredited Family Medicine Residency Program.
• Board Eligible required.
• Board Certification preferred.

• Experience with Electronic Health Records (EHR).
