Reason for Coverage: Need to help with long-term opening due to losing a provider

Work Schedule: 3-5 days per week

Hours: 12-hour shifts, 8:30 AM-4:30 PM, or 12:00 PM-8:00 PM

Average Patients Per Day: 16-20

Duties of Provider: Sick and well visits

Support Staff: RN & MA per MD
