SCHEDULE
- Coverage Type: Inpatient and Outpatient
- Schedule Type: Clinic and Call
- Call Type: Weeknight Beeper and 24 Hour Weekend Beeper Call
- Dates of Need: 1 week per month
- Shift Times: 8:00am-5:00pm (clinic) 8:30am-4pm (surgical center), 5pm-8am (night call), 24hr call (8am-8am)
- Lunch 1 hour
- Provider Min # Shifts Required: 1 week per month
- Guaranteed Hours per Day: 8
- Hours Worked for OT to apply: 9 per day
- Rates to be Filled Out on Bid: Clinic and Call: Regular, Orientation, Overtime, Holiday, Night Call, 24 Hour Call, Call-Back.

- Current CV (must be uploaded to bid)
- License: Active only
- DEA Required: Yes
- Boards: BC only
- Skills/Certs: BLS
ADDITIONAL DETAILS
Provider will work in clinic Monday, Tuesday, Wednesday and Friday. Surgical Center on Thursday's. They will be on call while they are in clinic which is apart of their regular hourly work. After that they will be on night call from 5pm-8am the next day. Weekend call would be 8am-8am.
Call is done via an app on their phone. They would take ED and established patient call. Provider's will need to be privileged at Providence for call only. Providence St. Vincent will be clinic and call work.
Evaluation of NICU airway required.
Interviews will be with the physician leader who is mainly available on Tuesdays between 1-2pm PST. It will be a teams interview.
